Welcome to NEVHC Health Education Department. The Health Education Department aims to be a leader in creating, offering, and evaluating patient and community education services. The department is part of Northeast Valleys vision to improve the health and well-being of patients and communities primarily serving the San Fernando and Santa Clarita valleys of Los Angeles County. The goal of the department is to promote healthy lifestyles, improve patient knowledge and skills to manage health conditions, and provide resources and referrals appropriate for enhancing health and wellness.

 
 
 


The Health Education department is staffed by a friendly, bilingual and bicultural team. Members are knowledgeable, friendly and dedicated to help individuals improve their health. They are specialized in a variety of topics such as chronic disease management; teen social and health issues intervention; lifestyle counseling and community outreach. Trained to meet the specific needs of Northeast Valley patients and community members, the Health Educators work closely with health center physicians, nurses, case managers, administrators, as well as community network liaisons to ensure patients have access to and receive the best service.

The department offers quality, cultural and linguistically appropriate services through:
Group classes
Individual phone counseling
Written materials such as booklets and handouts
Video, audiocassette, or computer technology
Community outreach
Health Fair participation

The Health Education department offers patient education classes at various primary care centers in English and Spanish. Some classes are even offered on Saturdays and are free of charge. Additionally, the department has for the past 11 years successfully managed the Information & Education (I&E) program. Through this state-funded program, Health Educators present trainings, workshops and resources at traditional and non-traditional school settings as well as community organizations. The focus is on adolescent social issues, pregnancy and sexually transmitted infection prevention, and access to quality health care.

Messages may be left in English or Spanish. Messages will be promptly returned. Patients are also encouraged to discuss any health education needs and interests with their medical provider during their clinical visit. If appropriate, the provider may refer the patient to any of health education service as part of their medical care.
