Responsibilities:
Maintains all aspects of the medical record department and patient record
system according to established clinic procedures and in accordance with
California and federal laws and regulations. Locates, pulls and charges
out patient records and makes available to the requesting provider/dept.
prior to the provision of any services to the patient. Institutes lost
or misfiled record searches and appropriate procedures. Locates, pulls
and charges out patient records for audit for administrative purposes
(e.g. audit, statistics, release or information). Completes new patient
charts and reassembles damaged charts. Delivers charts for patient appointment
schedule to requesting provider/department according to established time
schedule. Files medical record reports, forms and data into patient charts
and files patient records into record storage areas. Responsible for the
retirement of medical record charts. Reviews patient charts prior to releasing
to requesting provider/department for needed forms and reviews prior to
refiling to assure completion. Replenishes forms as needed. Returns records
to provider for completion when indicated. Transfers charts/records between
clinics according to established intra-facility transfer procedures. Requests
records from other providers and off-site storage. Processes all types
of requests for the release of patient medical record data including in
response to patient requests for access, other medical providers, subpoena,
etc., in accordance with state and federal laws and clinic procedures.
Operates office machines, including computer, photocopy machine,
mail machine and other office machines that may be required to perform
job. Participates in cross training exercises to assure competence in
the functions of Patient Registration Clerk and Telephone Operator and
functions in those positions as assigned/needed. Provides training, when
assigned, on departmental procedures. Maintains various logs and statistics.
Participates in all safety programs which may include assignment to an
emergency response team. Participates in hazardous waste and infection
control assignments as required in the health center which may include
being designated as an emergency responder to a hazardous substance release
or spill; performing infection control data collection, evaluation, reporting
and follow-up as specified in the NEVHC clinical health services policy
and procedures manual. Performs
other tasks as may be assigned by supervisor.

Qualifications:
High school diploma or GED.
Must have good written and verbal communication skills and reading comprehension.
Bilingual in English and Spanish required, with excellent written and
oral communication skills.
Must have pleasant, patient-oriented attitude and professional appearance.
One year previous experience in a clinic or office setting or completion
of equivalent job training program.
Available and willing to work a variable, flexible schedule a week (including
Wednesday nights and Saturdays).
