Definition:
Under the direction
of the Clinic Administrator and the Director of Nursing Services, the
R.N.- Coordinator organizes the provision of Clinical Patient Care Services,
in accordance with Federal, State, Agency, and various programmatic and
regulatory requirements. The R.N. Coordinator also serves as an integral
member of the interdisciplinary care team providing services to clients
receiving care at NEVHC health Centers. The RN Coordinator is responsible
for direct observation and interpretation of data to formulate a nursing
diagnosis through observation of the clients physical condition and behavior
and through interpretation of information obtained from the client and
others, including the health care team.

Responsibilities:
Is responsible for the supervision of the nursing staff, the annual evaluations,
training, scheduling, delegating work, and taking disciplinary action,
when needed. The RN Coordinator is responsible for direct observation
and interpretation of data to formulate a nursing diagnosis through observation
of the client's physical condition and behavior and through interpretation
of information obtained from the client and others, including the health
care team. Formulates a plan of care in collaboration with the client,
which ensures that direct and indirect nursing care services provide for
the clients safety, comfort, hygiene and protection, and for disease prevention
and restorative measures. Such formulation of care plan cannot be delegated
to an LVN, when indicated. Formulates and directs plan of care. The RN
Coordinator implements the nursing process based on direct assessment
and data collected from other members of the health care team. The RN
Coordinator provides care directly or delegates to subordinates based
on the legal scopes of practice of the subordinates and on the preparation
and capability needed in the tasks to be delegated and effectively supervises
nursing care being given by the LVN or MA per RN Standards of Competence.
Evaluates the effectiveness of the care plan through observation of the
client's physical condition and behavior, signs and symptoms of illness
and reactions to treatment and through communication with the client and
health team members and modifies the plan as needed. Acts as the client's
advocate, as circumstances require, by initiating action to improve health
care or to change decisions or activities which are against the interests
or wishes of the client, and by giving the client the opportunity to make
informed decisions about health care before it is provided. In collaboration
with the Director, Clinical Operations, the Medical Director, appropriate
Program Directors, and Clinic Administration; develops and implements
clinical policies and procedures to assure available trained personnel.
Assesses staff development and training needs and assists with in service
training for staff. Participates in the NEVHC Quality Management process
through the clinic's Quality Council. Assists in developing and implementing
a nursing quality improvement component to monitor the delivery of nursing
triage services in accordance with the established standards and procedures.
Assures compliance with nursing procedures relating to CHDP, Immunization
Program, BCCCP Program Indicators (e.g.: pap follow-up, immunizations,
anemia screening, nursing hypertension protocol), and similar special
programs. Conducts audits and reviews to monitor compliance. Prepares
or supervises the preparation of periodic reports, statistics, and studies.
Maintains patient follow-up procedures, including broken appointment and
abnormal lab follow-up procedures and immunization tickler/tracking system.
Assists in the development of and implementation of protocols and procedures,
and in the training of staff to enable staff to perform nursing education
in the nursing units and to maintain the competency levels of all clinical
staff, in accordance with corporate standards for clinical practice. Assists
in the training of staff to implement patient triage and scheduling system.
Functions as needed as a clinical nurse for the provision of providing
nursing support to the client and family to explain treatment and how
to care for the client's health needs, performing counseling services,
as well as triage, when required. Attends meetings as assigned by the
Director, Clinical Operations or Clinic Administration. Assists with the
implementation of clinic policies and procedures within the health center,
working in concert with other staff to resolve inter departmental issues,
recommending changes in policies and procedures as needed. Participates
in all safety programs which may include assignment to an emergency response
team. Participates in hazardous waste and infection control assignments
as required in the health center which may include being designated as
an emergency responder to a hazardous substance release or spill; performing
infection control data collection, evaluation, reporting and follow-up
as specified in the NEVHC clinical health services policy and procedures
manual. Performs other duties as required or as assigned by the Director
of Nursing Services or Site Administrator.

Qualifications:
Must be a graduate of an accredited school of nursing w/a valid, current
CA Registered Nurse license to practice.
Be able to communicate tactfully and empathetically, with cultural sensitivity;
with staff and patients. Fluently bilingual Spanish + English strongly
preferred.
Two years clinical experience as a Registered Nurse, with a minimum of
one year of experience in the clinical area assigned. Experiences must
have been as a Registered Nurse within a United States Licensing Jurisdiction.
Must demonstrate through written and/or oral testing, the knowledge and
skills necessary to provide care appropriate to the age of the patients
served and has knowledge of the principles of growth and development over
the life span. He or she is able to identify and categorize each patients
age specific grouping of needs, such as those for infant, adolescent,
adult or geriatric patients.
Must possess excellent interpersonal and supervisory skills.
Current, valid CA Driver's License, valid vehicle liability insurance,
and an available vehicle needed.
Must maintain a current Cardio-Pulmonary Resuscitation certification,
in compliance with corporate CPR policy.
Must
be willing and able to work a varied and flexible schedule to accommodate
the needs of NEVHC and its patients to include 8 a.m. to 5 p.m. (Variable)
Must
maintain or exceed competency levels as established by corporate standards
for clinical practice.
