Northeast Valley Health Corporation (NEVHC) has trained staff to help you apply for Medi-Cal (for children and pregnant women), Healthy Families and Healthy Kids programs. Below you can find out which health insurance programs you and your family may qualify for.
To apply for any of the programs listed below, please ask to speak with the “Consumer Relations” staff in the Business Office of your health center or call 1-866-MY-NEVHC (1-866-696-3842).
Do I qualify for…
Medi-Cal?
- This is a no-cost program for pregnant women and children up to 18 years of age.
- Documents needed to apply include:
- Proof of Income (paycheck stubs for the last month, unemployment or disability benefits, income tax return for self employment)
- Proof of Citizenship or Immigration (birth certificate, alien card, etc.)
- Pregnancy Proof (with expected date of delivery)
- Proof of California Residency (utility bills, rental agreement, etc.)
Healthy Families?
- This is a low-cost program for children up to 19 years of age who do not have insurance and do not qualify for low or no-cost Medi-Cal.
- The monthly premiums are between $5-$24 per child with a limit of $72 per family.
- There are no co-pays for preventive services, such as vaccines.
- There is a $5-10 co-pay for non-preventive services, like a sick visit.
- Documents needed to apply include:
- Proof of Income (paycheck stubs for the last month, unemployment or disability benefits, income tax return for self employment)
Healthy Kids?
- This is a low-cost program for U.S citizens and legal residents or undocumented children under 19 who do not qualify for Medi-Cal or Healthy Families.
- A co-pay of $5 is charged for most outpatient services such as doctor’s visits.
- There is no charge for preventive care such as vaccines and well child exams.
- Documents needed to apply include:
- Proof of Income (paycheck stubs for the last month, unemployment or disability benefits, income tax return if self employed)
- Proof of California Residency (utility bills, rental agreement, etc.)
