Northeast Valley Health Corporation (NEVHC) serves as the lead agency in managing one of the country’s largest Health Care for the Homeless grants for the Cooperative Health Care for the Homeless Network (CHCHN). Over the past twenty years, the CHCHN has worked with homeless healthcare partners to provide medical care, dental care, case management, behavioral health care services, alcohol/substance abuse treatment and outreach to those in need, regardless of their ability to pay for services. Network members also participate in UHHP (United Homeless Healthcare Partners), a community of service providers focused on issues surrounding the delivery of healthcare services to homeless residents of Los Angeles County.
The 14 health care agencies that make up the CHCHN provide complete health care services to at least 41% of the LA County homeless population at several sites throughout the county.
To ensure that the CHCHN providers are providing the highest quality care consistent with NEVHC’s mission, a Quality Improvement monitors and evaluates delivery of care across the city. The following indicators are reviewed:
Additionally, performance improvement projects conducted throughout the year assist in improving and maintaining the care that each agency provides to our homeless patients.
All patients, even those without a permanent address, have the right to obtain quality healthcare. NEVHC is proud to collaborate with a group of compassionate and dedicated service providers in this effort.
